Admission to Hospice

woman's hands writing on a clip boardA patient needs to meet certain physical eligibility criteria to qualify to receive hospice services. There are several ways for a patient to seek out the supportive services of our hospice team.

The patient or a family member can contact NHH and request an informational visit. During the informational visit our nurse can consult with both the patient and family and then call the doctor.

Clergy, friends, and others who care about a loved-one living with a life-limiting illness may also call NHH at any time to begin the referral process. The patient’s physician may often make the first call after consulting with the patient, but really anyone can call or even stop by our office if they know someone that might benefit from our hospice services.

After consulting with the patient and the family the rest of the referral process can be taken care of by our office. We often contact the patient’s personal physician to discuss a referral to hospice, or the hospice medical director can act as the patient’s primary physician.
